This form is for students registering for the first time with CDA. 

Welcome to the Center for Disability & Access (CDA) at the University of Utah. You will need to be a student and have an active uNID, CIS login and password to continue. 

If you are a non-student, or a former CDA student who needs to re-establish accommodations, please call us at (801) 581-5020 for assistance.

New CDA Students, please follow these steps:

1. Click the Register button above and complete the intake form.
        You’re welcome to upload any documentation you have to support your request. Submitting documentation before your appointment is helpful, but not required. If you don’t have documentation available or would rather wait until your appointment, that’s perfectly fine.

2. After completing the registration form, you will receive an email with scheduling instructions within 2-3 business days.
